Job Title: Licensed Practical Nurse (LPN)

Description:
The LPN ratios are 2 LPNs per shift.
One LPN will be assigned to memory care (15 beds) and one will manage skilled care (15 beds).
They will split time assisting with an additional 20 assisted living beds.
The maximum patient load is 25.
For day shifts, the goal is to have three LPNs working.
Common diagnoses include CHF, hypertension, diabetes, history of strokes, Alzheimer's, dementia, and total cares.
Responsibilities will include administering tube feedings, blood sugar checks, insulin, vaccinations, oral, subcutaneous, and intramuscular medications, along with skin assessments.
The director will manage admissions, while discharges are not applicable as residents stay for life.
Charting will be done by exception.

LPNs must be flexible and willing to assist with hands-on care alongside Certified Nursing Assistants as needed.
Common IV medications administered are rare, typically only IV antibiotics.
Visiting nursing services will be used for IV administration when necessary.
A supervisor or assistant supervisor, both of whom are RNs, will be present on each shift.
Night shift staff can communicate with shift supervisors during overlap.
Respiratory equipment used includes O2 and CPAP; there are no tracheostomies or chronic ventilators.
Breathing treatments and inhalers are utilized for respiratory care.
All shifts are 8 hours: 6 AM - 2:30 PM, 2 PM - 10:30 PM, and 10 PM - 6:30 AM.
Flexibility in scheduling is required, particularly for vacation or family medical leave adjustments.
Weekend work is required every other week, with holiday coverage as needed.
LPNs will be trained on all units throughout the facility.
Scrub color is flexible, but solid colors are preferred.
Unit-specific orientation may be required prior to full assignment.
Personal appearance guidelines include no facial piercings, no offensive tattoos, and no visible tattoos on the face or neck.
Smoking of any kind is prohibited on the premises.
Use of earbuds or personal phones in patient care areas is not allowed.
Breaks are designated during shifts.
