As a Registered Nurse (RN) in Community Care Services, you will play a vital role in providing comprehensive nursing care to patients in a hospital settings. You will assess patient needs, develop and implement care plans, and ensure the delivery of high-quality, patient-centered care. Your work will directly support patients and their families, helping them achieve their health goals and improve their quality of life.

What You Will Do

• Conduct admission and ongoing assessments of patients using physical and psychosocial assessment skills, diagnostic data, and medical evaluations.
• Evaluate the effectiveness of care provided in relation to short and long-term patient goals.
• Develop and implement individualized patient care plans, anticipating and addressing patient needs.
• Execute nursing actions and medical orders to meet the complex needs of patients and their families, considering age-specific psychosocial and educational requirements.
• Document care provided and patient responses accurately in the patient's record.
• Assume Charge Nurse responsibilities, overseeing LPNs and non-licensed technical personnel, and demonstrating leadership through role modeling, communication, and problem-solving.

• Graduate of a state-approved school of nursing.
• Current State of Michigan RN licensure.
• At least 12 months of nursing care experience, including 3 months of experience providing nursing care to maintenance Hemodialysis patients, prior to assuming charge responsibilities.

• Previous dialysis experience.
