Volunteer AmeriCorps Patient Health Navigator

Patient Health Navigator Program Sea Mar Community Health Centers, a Federally Qualified Health Center founded in 1978, is committed to providing comprehensive health, human, housing, educational, and cultural services to diverse communities, with a special focus on serving Latinos in Washington State. Sea Mar offers an AmeriCorps Patient Health Navigator position as a full‑time volunteer service with a modest living allowance stipend. Program Overview Patient navigators work with health services providers to assist patients in accessing appropriate care, establishing a medical home, and ensuring that pre‑ and post‑visit needs are met. Navigators also facilitate brief health education encounters for chronic conditions, complicated medical cases, and patients with multiple barriers to clinical care. Translation/interpretation services may be required. Essential Duties and Responsibilities Schedule appointments and guide patients through health system barriers. Initiate communication with patients and assist with referrals to address comprehensive needs. Participate in mandatory trainings, service projects, and team‑building activities throughout the 10–11 month term. Create informational materials and resources for Sea Mar sites, including posters, handouts, recipe guides, and exercise information. Develop a community resource guide for patients and the AmeriCorps‑Sea Mar group. Review electronic health records to identify patients in need of screenings and schedule appropriate appointments. Prepare mailouts with relevant materials and patient identifiers. Collaborate with clinic staff, quality improvement teams, and other departments to identify patients requiring screenings and inform care teams. Support roll‑out of preventive health screening and recall efforts. Lead or participate in AmeriCorps site health projects focused on smoking cessation, diabetes education, nutrition, healthy lifestyles, stress reduction, chronic‑condition management, and adolescent topics. Promote health coverage enrollment opportunities through community outreach and event participation. Assist in developing and distributing informational materials and establishing referral relationships with local organizations. Qualifications Bilingual Spanish skills strongly preferred. Reliable transportation required. Compensation and Benefits Salary plan of $850.00 USD biweekly (modest living allowance stipend). After successful completion of service, a maximum $7,395 Education Award may be awarded.
