The ideal candidate will have recent home health experience, be proficient with EPIC EMR and OASIS documentation, and be comfortable managing both Start of Care (SOC) and revisit patient appointments. Responsibilities\n \n Evaluate and treat patients in their homes to improve mobility, strength, balance, and functional independence.\n Perform comprehensive Start of Care (SOC) assessments and follow-up (revisit) therapy sessions.\n Complete accurate OASIS assessments and required clinical documentation.\n Develop and implement individualized treatment plans based on patient needs and physician orders.\n Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.\n Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members.\n Document all patient care accurately and timely in EPIC EMR.\n Monitor patient progress and modify treatment plans as appropriate.\n Ensure compliance with CMS regulations and Penn Medicine Home Health policies.\n Required Qualifications\n \n Active Physical Therapist (PT) license in New Jersey.\n Recent and relevant Home Health Physical Therapy experience is required.\n Experience with Start of Care (SOC) and revisit home health visits.\n Proficiency with EPIC EMR.\n OASIS documentation experience is required.\n Current BLS Certification (AHA preferred).\n Valid driver's license, reliable transportation, and auto insurance.\n Strong clinical assessment, communication, and documentation skills.\n Preferred Qualifications\n \n Experience managing an independent home health caseload.\n Strong time management and organizational skills.\n Excellent patient education and interdisciplinary collaboration experience.\n Key Skills\n \n Home Health Physical Therapy\n Start of Care (SOC)\n Revisit Visits\n OASIS Documentation\n EPIC EMR\n Patient Assessment\n Rehabilitation\n Care Planning\n Patient Education\n Clinical Documentation\n Interdisciplinary Collaboration\n Must-Have Skills\n \n Active NJ PT License\n Recent Home Health Experience\n OASIS Experience\n EPIC Experience\n Start of Care (SOC) & Revisit Experience\n Reliable Transportation and Valid Driver's License\n \n \n TalentBurst, Inc Job ID #26-25158.
