Acute Rehab Case Manager: Patient Care Leader

Kent Hospital, located in Warwick, RI, is part of Lifepoint Rehabilitation. We are seeking a Licensed Case Manager for the Acute Rehabilitation Unit (ARU) to coordinate patient care, plan discharges, and collaborate with the interdisciplinary team. Ideal candidates hold RN or social work licensure (LMSW/LCSW/LICSW) with inpatient case management experience; CCM/ARN/ACM/RN-BC certification is a plus.
