At Jefferson House we provide excellent long-term care and rehabilitation services to seniors in the greater Hartford area for over 120 years. We are comprised of a skilled and caring staff which provides seniors with Nursing Services, Therapeutic Recreation, Spiritual Services, and Social Services, aimed at caring for the whole individual. 

Monitors and maintains a focus on patient outcomes for continuous quality assurance and performance improvement. Supports community wide staff education in areas that are linked to quality assurance and performance improvement (QAPI) across all departments. Conducts educational sessions focused on improving clinical outcomes. Focuses on both patient and process outcomes for continuous quality improvement. Designs and monitors indicators using SCC’s QAPI process to generate accurate and timely reports. Reviews all incident reports and exercises professional judgment in assuring patient confidentiality as a high priority. Demonstrates competencies to meet age specific patient and resident needs as evidenced by positive outcomes with focus on customer service, culture change and documentation.

Qualifications

• Active Registered Nurse (RN) in the state of Connecticut
• Two years clinical experience in a Skilled Nursing setting
• On year of nursing supervisory experience in a Skilled Nursing setting
• Technical ability to work with and interpret large amounts of data to develop solutions

• Ability to teach, coach and mentor nursing staff
