Oak Street Health is seeking a compassionate Primary Care Physician to deliver high-quality, preventative care to an adult Medicare population in underserved communities. You will manage a dedicated panel of patients, perform comprehensive primary care visits, coordinate chronic disease management, and lead care planning in collaboration with nurses, care managers, and social workers. The role emphasizes patient-centered, value-based care, population health, and closing care gaps. You’ll practice in a supportive, team-based environment with time for meaningful visits, quality improvement, and professional growth.
