Summary of Essential Functions Provides intensive, time-limited, community mental health case management to participants in the Permanent Supportive Housing program who are transitioning from hospitalization or homelessness to permanent housing within the Staunton/Waynesboro, Augusta, and Highland County catchment areas. Services include, but are not limited to: a minimum of a weekly in-person face-to-face contacts, referral to other agencies, supportive counseling, outreach, crisis intervention, and assessment. Collaborates with PSH Specialist to promote housing stability. Provides comprehensive initial and annual assessment of client strengths and needs. Participates in development of the consumer service plan. Refers, links, and coordinates Medicaid and non-Medicaid services. Provides supportive counseling and case management services. Provides crisis intervention and crisis stabilization services as needed. Requirements Bachelor's degree in human services or related field. Previous work or internship experience that provides the necessary knowledge, skills, and abilities. Must pass required pre-employment screenings, including, but not limited to, criminal history and experience verification. Current Virginia driver’s license with zero points or more.
