Field-Based LTSS Clinician and Care Coordinator

Elevance Health is seeking a LTSS Service Coordinator-Clinician in Irving, Texas. This field-based role will require travel to client sites and focuses on assisting RNs/LSWs in coordinating care for members with chronic illnesses. Ideal candidates will have relevant licenses such as LSW or LCSW and at least 2 years of related experience.
