Job Description

Registered Nurse (RN) – Hospice Case Manager | Wisconsin

Join a compassionate hospice team providing exceptional in-home care to patients throughout Fond du Lac, Dodge, Columbia, and Winnebago Counties , with additional opportunities in Brookfield, Jackson, Waterford, and Sheboygan .

Schedule

Monday–Friday, 8:00 AM – 5:00 PM
No weekends or on-call
Approximately 5 patient visits per day
Mileage reimbursement: $0.43/mile

Responsibilities

Provide skilled hospice nursing care and comprehensive patient assessments.
Develop and manage individualized care plans.
Perform wound care, pain and symptom management, and patient/family education.
Coordinate care with physicians and the interdisciplinary hospice team.
Complete accurate EMR documentation and participate in care planning and recertifications.

Requirements

Active Wisconsin RN license
CPR/BLS certification
Wound care experience
Valid driver's license, reliable vehicle, and auto insurance
Home health or hospice experience preferred
HCHB experience preferred (MatrixCare or other EMR experience accepted)

Training Available

Candidates without HCHB or hospice experience are welcome to apply. Paid training is provided based on experience, including HCHB training for experienced hospice nurses and additional hospice education for qualified home health or skilled nursing candidates.

Compliance Requirements

Candidates must complete required onboarding, including a skills competency checklist and training in HIPAA, OSHA, Infection Prevention & Control, Elder Abuse, and other required clinical competencies.
