The Clinical Pharmacist is responsible for identifying, preventing, and/or resolving medication related problems; for providing authoritative information to other health care professionals, patients, and the public; and for assuring that the informational, clinical, and drug distribution components of pharmacy services function as an integrated system to provide patient centered care to the patients and staff of Northwestern Medicine.

Responsibilities include interpreting medication orders to ensure safety and proper use of medications, monitoring medication therapy for significant drug interactions, evaluating and documenting that the medication(s) selected are appropriate for achieving the desired therapeutic outcome, acting as a consultant in the areas of pharmacokinetics and adverse reactions to physicians, nurses and other healthcare professionals, processing all medication order information into the Electronic Health Record to ensure accuracy of records regarding past, current and future dispensing and charging, maintaining a comprehensive pharmacy patient profile, overseeing the packaging labeling, and distribution of medications for all patients and employees according to established policies, procedures, and guidelines, compounding and dispensing pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products, verifying medication orders before delivery to patient care areas and ensuring accuracy and completeness, reviewing all laboratory data for appropriateness of medication use and recommending changes when needed, developing and presenting orientation, education and training programs for medical, nursing, hospital and pharmacy staff, as well as assisting in the educating of pharmacy students, residents, and fellows, educating patients and their families about specific medications, responding to emergency situations to assist with medication management, providing accurate and timely medication information and or teaching to healthcare professionals or patients, maintaining the hospital formulary and acting to limit medication usage to formulary items when appropriate, documenting all clinical activities and interventions performed, communicating professional knowledge effectively in both oral and written forms to patients and other health professionals, participating in quality improvement and medication use review activities of the department, organizing, prioritizing, and planning workload within given time constraints, reviewing restocking automated dispensing cabinets throughout the hospital and resolving any problems, ensuring compliance with hazardous waste collection and disposal procedures, maintaining proficiency and expertise in area/s of practice through continuous professional development, and all other job duties as assigned.

This position is full-time, 80 hours per pay period, 8-hour rotating shift: Monday through Friday hours start times dependent on area of coverage (7:00 a.m., 7:30 a.m., 8:00 a.m.), every 3rd weekend (Saturday and Sunday) start times 1:30 p.m. or 2:30 p.m., 2 holidays per year Looking for experience in ER coverage, centralized and decentralized operations.

Qualifications

Minimum Requirements: Education: Bachelor's Degree in Pharmacy or a Doctor of Pharmacy Degree Licensure: Active Clinical Pharmacist license in the state of Illinois (IDFPR)

Preferred: Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.) Completion of a pharmacy residency program or equivalent hospital pharmacy experience
