Nurse Case Manager II

Job Title: Nurse Case Manager II
Job Duration: 3 Months (Possibilities of extension)
Shift: Mon to Fri, 8 am to 5 pm
Job Location: Charlotte NC 28201

Will consider submissions outside of NC if:
1. they have a compact or NC RN license.
2. they must reside in the EST time zone.

They are required to have a private office space free of any distractions due to HIPPAA Compliance.

REQUIRED:
Must have experience working a remote position previously.
Must have case management experience.

Duties:
• 50-75% of the day is dedicated to telephonic engagement with members and the coordination of their care.
• Compiles all available clinical information and partners with the member to develop an individualized care plan that encompasses goals and interventions to meet the member's identified needs.

Experience:
2-3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.
2 years Healthcare and/or managed care industry experience.
Case Management experience required--
Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding
Effective communication skills, both verbal and written.
Ability to multitask, prioritize and effectively adapt to a fast paced changing environment
Sedentary work involving periods of sitting, talking, listening. Work requires sitting for extended periods, talking on the telephone and typing on the computer.
Work requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitor.
Typical office working environment with productivity and quality expectations
North Carolina RN license

Education:
RN with current unrestricted state licensure.
Case Management Certification CCM preferred
