Case Coordinator
St. Vincent's Medical Center
St. Vincent’s Medical Center, part of Hartford HealthCare, seeks a compassionate Case Coordinator to help patients navigate care and connect with essential services. In this role, you will coordinate referrals, appointments, follow-up, and communication among patients, families, healthcare providers, and community partners. You will help identify barriers to care, maintain accurate confidential records, and support individualized care plans in collaboration with multidisciplinary teams.
