As a Case Manager RN, you will coordinate patient care, ensuring smooth transitions and effective discharge planning. This role requires a strong background in acute care and familiarity with Epic for charting. Expect a schedule of 5x8 days, with weekends required on alternating weeks. • Must have AK RN license in hand for Auto Offer
• References required: 2 covering 1 year within the last 3 years
