This is a full time position that will service patients across Scottsdale and Fountain Hills!

Annual salary range $85000 - $95000 per year (depending on experience), plus travel expenses.

As a Home Health RN, you'll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:

• Build meaningful relationships with patients and families
• Create personalized care plans that truly make an impact
• Coordinate care as a trusted Case Manager
• Educate and empower patients and caregivers with confidence

Requirements

• Current RN license in the State of Arizona
• Valid driver's license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and make a real difference
