Primary Care Nurse Practitioner or Physician Assistant

Optimal Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in a patient-focused practice. In this role, you will perform histories and physicals, diagnose and treat acute and chronic conditions, and develop evidence-based care plans for diverse adult populations. Responsibilities include ordering and interpreting labs and imaging, prescribing medications, documenting in an EMR, and coordinating care with physicians and specialists. You will educate patients on prevention and lifestyle management and participate in quality improvement.
