Licensed Practical Nurse (LPN)
The Licensed Practical Nurse (LPN) is responsible for providing comprehensive, patient- and family-centered nursing services in coordination with a multidisciplinary care team. The LPN performs basic clinical assessments, patient education, care coordination, and clinical support activities within an established scope of practice under the supervision of a licensed provider. This position operates primarily within a primary care and family practice setting, supporting patients with acute, complex, and chronic conditions.
Provides basic triage nursing assessments for patients presenting via telephone, patient portal, or directly in clinic.
Consults with the medical provider and/or RN as needed regarding patient status to determine the appropriate level of care.
Schedules same-day appointments in coordination with the provider.
Delivers appropriate patient education and home-care guidance within LPN scope of practice.
Obtains and communicates information for patients requesting laboratory/test results and medication refills.
Accurately documents assessments, patient education, nursing interventions, and encounter notes in the Electronic Medical Record (EMR) in a timely manner.
Under provider oversight, reviews and processes medication refill requests and initiates prior authorizations as required by payers.
Performs routine direct patient care within the LPN scope of practice, including but not limited to:
- Administering injections, immunizations, and medications as prescribed.
- Performing dressing changes and basic wound care.
- Obtaining and recording vital signs, height, weight, BMI, and pain assessments.
- Collecting specimens (e.g., blood draws, urine, and other samples) for laboratory processing.
- Conducting point-of-care testing as directed.
- Providing patient and family education on treatments, disease processes, health promotion, and preventive care.
Provides education on disease processes, prevention strategies, health promotion, and chronic disease self-management (e.g., diabetes, hypertension, asthma, COPD, and obesity).
Collaborates with members of the multidisciplinary care team — including Nursing, Community Health, Nutrition, Behavioral Health, and Pharmacy — to identify and address barriers to achieving patient health goals.
Under the supervision of an RN, assesses patient risk, monitors and evaluates changes in condition, coordinates complex patient care, facilitates referrals, and performs routine follow-up.
Administers medications and provides education and coaching to support patients in effectively managing their health conditions.
Coordinates and communicates various direct and indirect patient care activities, including collaboration with:
- Pharmacy partners
- Specialty and ancillary referrals
- Durable Medical Equipment (DME) suppliers, Home Health agencies, and Physical/Occupational organizations
- Insurance/payer organizations
- Healthcare organizations and hospitals
- Schools and community organizations
- Other community partners and social services agencies
Participates in daily huddles and pre-visit planning with the provider and care team to prepare for patient encounters and proactively address care needs.
Contributes to the development, maintenance, and updating of educational materials and patient teaching tools in collaboration with the multidisciplinary team.
Supports team-based efforts to improve performance metrics established by the Health Resources & Services Administration (HRSA), the Primary Care Health Home (PCCH) model, Accountable Care Organization (ACO) requirements, and other regulatory or partnership-based quality frameworks.
May participate in learning collaboratives, quality improvement sessions, and partner calls to facilitate and share best practices within the nursing role.
May utilize clinical data platforms (e.g., DRVS, Cyber Access, or equivalent reporting tools) to monitor the effectiveness of individual care plans, particularly for patients with Medicaid and other coverage populations.
Provides essential support and assistance to nursing team members to ensure consistent, high-quality patient care and team success.
As a member of the healthcare team, responds promptly to and appropriately to in-office medical emergencies.
Serves as a clinical resource for Medical Assistants and other support personnel as needed.
Attend departmental, site, and organization meetings as required.
Participate in process improvement projects, problem-solving groups, and quality initiatives.
Contribute to data analysis and quality assurance activities.
Engage in organizational patient safety programs and reporting systems.
Perform other duties as assigned by Nursing Director or designee.
Graduate of an accredited school of practical nursing required.
Current, active licensure as a Licensed Practical Nurse (LPN) in the State of Missouri, in good standing with the Missouri State Board of Nursing, required.
Minimum of two (2) years of nursing experience, preferably with patients with complex conditions requiring care coordination and multidisciplinary collaboration.
Experience in a primary care, family practice, or Federally Qualified Health Center (FQHC) setting preferred.
Obstetrics (OB) experience preferred.
Current Basic Life Support (BLS/CPR) certification required (American Heart Association or American Red Cross).
Must meet all background check, health screening, and immunization requirements per FCHC policy.
Regularly required to stand, walk, sit, talk and hear.
Frequently required to use hands and fingers to handle, feel or operate equipment.
Occasionally required to reach, stoop, kneel, crouch, or crawl.
May occasionally be required to lift or move up to 50 pounds.
Work environment involves frequent exposure to blood-borne pathogens, communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinical healthcare setting.
Personal Protective Equipment (PPE) is required in applicable situations per OSHA and FCHC safety standards.
Required to satisfy continuing education requirements as established by the Missouri State Board of Nursing for license renewal. FCHC also requires participation in mandatory organizational training (including annual HIPAA, safety, and compliance training), and encourages engagement in professional development opportunities relevant to the primary care and FQHC setting.

The employee must comply with all applicable federal and state privacy regulations, including the Health Insurance Portability and Accountability Act (HIPAA) and the Family Educational Rights and Privacy Act (FERPA), in all aspects of client care, communication, and recordkeeping within the school-based health setting.
