Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Conduct clinical assessments to determine patient needs and evaluate appropriateness for therapy.
• Prepare and maintain accurate documentation, including Plans of Care (POC) and progress notes, and revise based on ongoing assessments.
• Develop and implement treatment plans for neuromuscular, neurological, and orthopedic conditions, in accordance with medical referrals.
• Educate patients and families on therapeutic procedures and assist with the safe use of equipment.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.
• Ensure compliance with infection control and safety protocols during patient care activities.
