Job Title Physical Therapist Location Texas | Outpatient Schedule / Job Type Contract | Full-Time | Days | Onsite | 40 hours/week Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Conduct 60-minute evaluations and treatments, utilizing clinical reasoning to progress patients toward discharge within an established length of stay Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing Required Qualifications Active Physical Therapist license in good standing in the state of Texas Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans Proficiency in medical record documentation in compliance with state and federal guidelines Commitment to patient and employee safety standards and organizational compliance requirements Completion of all state-mandated continuing education requirements Preferred Experience / Skills Prior experience in an outpatient physical therapy setting Experience supervising or mentoring physical therapy students Demonstrated involvement in professional development activities, continuing education, or professional organizations Why Join Us Work in a collaborative outpatient environment with a strong emphasis on quality care, patient safety, and professional growth Opportunity to actively shape your own professional development plan with clinical management support Concentric Healthcare Staffing Job ID 775982.
