RN Hospice On-Call Nurse (Part-time)

RN Hospice Nurse - Home Visits Position is based in Lakeland, FL and requires daily travel to assigned areas in Polk County. Schedule: flex, min. 20-24 hrs per week. Overview Compassionate Care Hospice (CCH), an Amedisys Company What We Offer Competitive pay (\"$38-\$42/hr\") and flex schedule In‑depth orientation and training process Opportunities for professional growth A supportive, team‑oriented environment Technology Used HCHB = Proprietary real‑time database Enclara Pharmacy solutions with 24/7 access IPrescribe – efficiencies with medication management Forcura – document management system Medline - supply ordering system TalentEDGE - Amedisys' premier learning center EBSCO - nursing resource center for CEUs Tablet with voice dictation and mileage tracking Outlook added to Teams, Chat, Yammer Responsibilities Perform initial nursing assessment on new admissions Complete, coordinate and revise the total plan of care and maintain continuity of patient care by collaborating with appropriate staff Regularly communicate patient progress to the clinical manager, physician and care team Develop, prepare and maintain individualized patient care progress records with accuracy in a timely manner and according to care center policies Record pain/symptom management changes and outcomes as appropriate Educate families on patient condition, needs and care Make referrals to other hospice care disciplines as needed or document rationale for not doing so Perform on‑call responsibilities and services to patients/families as assigned Perform other duties as assigned Qualifications Required Current RN license in the state(s) where you will work Minimum 1 year of clinical RN experience in an acute healthcare setting Current CPR certification Preferred Hospice or home health experience Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities Proficiency in electronic medical record (EMR) systems Ready to take the next step?
