Responsibilities Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses Educate patients and families on disease management, self-care, and wellness strategies Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services Maintain accurate and detailed records of patient interactions, assessments, and care plans Requirements Current RN license in the state of Wyoming Minimum of 0 years of experience in home care or a related setting (new graduates welcome to apply!)
