Role As an Ambulatory Clinical Pharmacist, you will be part of the Centralized Prescription Renewal team. You will specialize in collaborating with providers and community pharmacies to process prescription refills for your patients. As a Pharmacist you will follow a protocol. Based on whether the request meets criteria and on clinical judgment, you will either approve or deny a renewal request. Additionally, you will have the ability to facilitate ordering labs and appointment scheduling. Knowledge in chronic disease state management, including guidelines, treatment, and monitoring parameters is highly preferred. We are looking for someone to join our team, per diem coverage for PTO, sick leave and unexpected absences, averaging 2-4 days per month. Training will be 3 weeks (3 days per week minimum). Completion of post‑offer occupational health physical assessment, drug screen, and background check (includes employment, criminal, and education) is required. Position Summary This position actively participates on the healthcare team as a pharmacotherapy expert in ambulatory care to provide high‑quality direct‑patient care and serve as a resource and drug information specialist to all other healthcare staff. You will contribute to the optimal selection of drug therapy, design, and implementation of patient‑specific medication regimens to maximize therapeutic benefit, minimize toxicity, and attain quality measure gap closure. You will accurately and completely document patient assessment data and clinical interventions, provide comprehensive patient education to improve medication compliance, and promote patient self‑management skills. You must possess clinical acumen and the skills necessary to assess and interpret data reflective of the patient’s clinical condition. You will conduct educational activities, apply principles of performance improvement, and may participate in committee activities as appropriate, assisting with the development and implementation of evidence‑based medication guidelines/protocols. You may oversee residents, interns, pharmacy technicians, and support staff as applicable, providing direction in completion of daily responsibilities.
CORE FUNCTIONS
Use best practices and knowledge of internal or external business challenges to suggest improvements to services, processes, or products. Provide oversight and assist with the development and implementation of evidence‑based collaborative practice protocols. Participate in Ambulatory Pharmacy and Therapeutic (P&T) or other system committee activities as appropriate. Evaluate and recommend medications for formulary. Prepare formulary evaluations for P&T consideration. Solve complex problems. Take a new perspective using existing solutions. Perform medication therapy and disease state management, which includes critically evaluating disease states and adjusting medication therapy per the ambulatory pharmacy collaborative practice protocols to optimize patient regimens and improve clinical outcomes. Ensure utilization of appropriate and cost‑effective medication therapy. Focus areas may include, but are not limited to, disease state management (e.g., thromboembolic disease, diabetes, cardiovascular disease, COPD), medication management, adverse drug event prevention and management, and evaluating and addressing quality measure therapy gaps. Interpret customer needs, assess requirements, and identify solutions to non‑standard requests. Interact primarily with department and cross‑department peers, supervisor, customers, peers’ managers, patients, and physicians. Function as a therapeutic consultant in areas such as rational and appropriate use of medications, clinical pharmacology, pharmacokinetics, and disease state management. Demonstrate accurate and thorough documentation skills. Facilitate pharmacist‑to‑provider communication and clinical interventions. Make decisions within approved operating plans and objectives and within functional policies and precedents. Determine how and when to achieve results. Participate in ordering or prescribing, administering, and monitoring of all medications utilized. Internal customers may include patients, nurses, licensed independent practitioners, administrators, and other health‑care team members to integrate services, improve patient care, and ensure effective communication systems. External customers may include local, state, and national pharmacy organizations, State Board of Pharmacy, colleagues and peers, educational institutions, and pharmaceutical representatives. Minimum Qualifications A degree in Pharmacy from an accredited College of Pharmacy. Completion of an accredited PGY1 pharmacy residency program or three years of pharmacy experience. Requires current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire date. A current and unrestricted pharmacist license (not intern) is required at the time of hire for BSS applicants. Incumbents in Arizona must provide the AZ Board of Pharmacy Wallet Card at time of hire. Maintain good organizational, problem‑solving, and computer skills. Possess good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts. Preferred Qualifications Previous clinical experience in the ambulatory care setting; additional related education and/or experience, including board certification in a related field, or PGY2 pharmacy residency program in ambulatory care.
