Hospice Registered Nurse Case Manager

Capital City Hospice is seeking an RN Case Manager to join a compassionate Central Ohio team. This full-time role offers Monday–Friday scheduling, low caseloads, and no routine on-call to support work-life balance. You’ll assess patients, determine hospice eligibility, lead admission planning, and coordinate multidisciplinary care with physicians, nurses, social workers, and aides to ensure dignified end-of-life care for families. #J-18808-Ljbffr
