Overview The Community Health Worker functions as the frontline public health worker meeting all competencies and objectives of exhibit C of the Community Health Worker contract with the Missouri Primary Care Association (MPCA). The CHW is also responsible for providing comprehensive evaluation and recommendation of healthcare coverage options to meet the needs of community members and providing application assistance for Medicaid, Medicare, Market Place, and other insurance programs. This position is an organizational level position addressing the healthcare coverage needs of all Four Rivers patients and community members. Supervision Non‑supervisory and reports directly to the Care Management Manager. ESSENTIAL POSITION RESPONSIBILITIES The Community Health Worker (CHW) will be responsible for helping patients navigate and access community services and adopt healthy behaviors. The CHW assists eligible clients to apply for publicly sponsored health care insurance plans, providing continuous follow‑up throughout the approval process. The CHW conducts outreach in the community that promotes, maintains, and improves individual and community health, provides information on available resources, offers social support and informal counseling, and advocates for individuals and community health needs. The CHW collects data to identify community health needs and conducts continuous process improvement as part of the quality assurance program. The CHW conducts professional duties in accordance with The Community Health Center’s (YCHC) mission, vision, and values. Position Duties Establish trusting relationships with community partners, patients, and their families while providing general support and encouragement. Contact prospective program participants by mail, telephone, or in person to inform them of available services, explain the impact of the program, and encourage participation. Assist families, groups, and communities in accessing health‑related services. Assess clients’ eligibility for services such as financial assistance, insurances, and other assistance programs. Assist eligible clients to apply for publicly sponsored health care insurance plans, providing necessary follow‑up, tracking, and data collection to assure continuous enrollment. Assess client needs and refer clients to community resources and other agencies to meet identified needs. Attend community meetings and appropriate health fairs to understand community issues and provide information to community members. Identify the particular health care needs of individuals in the community. Advocate for individual or community health needs. Act as a liaison between communities, individuals, and other service providers. Help clients understand their health conditions, develop strategies to improve their health conditions, and provide informal counseling and support. Help improve community residents’ ability to effectively communicate with health care providers and reduce cultural and socio‑economic barriers between patients and providers. Distribute posters, flyers, and brochures to YCHC service areas appropriate for the target population. Utilize electronic health records, Excel spreadsheets, word processing, and other software programs to format documents, spreadsheets, brochures, flyers, newsletters, training materials, and other documents. Produce technical graphics, charts, and tables, selecting the format that best displays the material. Enter data and produce various reports as required under the Community Health Worker contract with MPCA and in accordance with the Primary Care Health Home (PCHH) program requirements. Minimum Qualifications High school diploma or equivalent required. Experience with social services or public health systems is preferred.
