Must-Have: PT Experience, Outpatient Experience, Supervisory Reference Forms completed within the last two years

Description: This position delegates responsibility to the Physical Therapist Assistant and aides. Activities of this position are patient-focused and utilize assessment, planning, intervention, and evaluation in accordance with physical therapy standards, physician orders, and governing policies and procedures. The Physical Therapist evaluates the patient's function by performing formal and informal testing measures to determine skeletal, neurological, respiratory, and cardiovascular status. The Physical Therapist develops a goal-oriented treatment regimen based on assessment data and the patient's tolerance to carry out the treatment plan as prescribed. A written treatment regimen is submitted to be included as part of the plan of treatment authorized by and coordinated with the physician. The plan of care is updated as needed to reflect changes in patient status. The Physical Therapist co-signs verbal orders received by the physical therapist assistant as necessary. A schedule of treatment, which is frequency and duration specific, is devised to be discussed with the patient, family, and caregiver while coordinated with clinical support staff. Education is provided to the patient, family, and/or caregiver regarding pathology or condition, patient alternatives, prognosis, and rationale for treatment through forms of rest, exercise, modalities, and other indicated interventions.
