Home Health Aide

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Home Health Aide (HHA) – In-Home Care

REQUIRED TO APPLY: Applicants must have a full, valid driver’s license held for at least 12 months , a reliable personal vehicle , and a valid HHA or CNA certificate .

Location: Plymouth & Surrounding Areas

Wage: $19.00/hour + Mileage Reimbursement

Schedule: Flexible Hours | Based on Availability

This position starts on a per diem (as-needed) basis , with the opportunity to gradually build your schedule as you pick up additional clients.

Join our team as a Home Health Aide (HHA) and provide meaningful, one-on-one care to individuals in their homes while supporting their independence, well-being, and quality of life.

Qualifications

• Valid and current HHA or CNA certificate

• Full, valid driver’s license held for at least 12 months (permits and JOLs do not qualify)

• Personal reliable vehicle for traveling to individuals’ homes

• Must be at least 18 years old

• High school diploma or equivalent

• Must pass required background checks, physical exam, and TB screening

• Comfortable using a mobile device for documentation

• Caregiving experience preferred

• Compassionate, dependable, and a strong communicator

Don’t have an HHA/CNA certification? Other qualifying certifications are welcome and may be eligible for the following pay rates:

• Homemaker (HM): $18.00/hour

• Personal Care (PC): $18.00/hour

• Personal Care/Homemaker (PCHM): $18.50/hour

What You’ll Do

• Provide in-home ADL and personal care support, including bathing, dressing, grooming, mobility, and transfers

• Assist with light housekeeping, meal preparation, errands, and grocery shopping

• Monitor health needs, document services, and maintain a safe, supportive home environment

• Build positive, trusting relationships with the individuals you serve
