Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans in a skilled nursing facility setting.
- Provide therapeutic interventions such as therapeutic exercises, gait training, balance work, ROM, strength training, and functional mobility activities.
- Monitor progress, document outcomes, and adjust plans to meet goals while maintaining regulatory and documentation standards.
- Collaborate with the interdisciplinary team to coordinate care, discharge planning, and progression of therapy plans.
- Educate residents and families on home exercise programs, fall prevention, and safety strategies.
- Maintain appropriate infection control, safety practices, and compliant patient records.

Requirements
- Active physical therapy license in New Hampshire (or eligibility for practice in the state)
- Doctor of Physical Therapy (DPT) or equivalent degree
- Ability to work effectively in a long-term care setting and collaborate with a multidisciplinary team
- Basic documentation skills and familiarity with therapy charting; strong communication skills
- Certification in CPR/BLS preferred
- Prior experience in a skilled nursing facility or rehabilitation setting is advantageous
