Clinical Population Health Pharmacist - Value‑Based Care

Banner Health is seeking a Population Health Clinical Pharmacist to support value-based care initiatives within the Insurance Division. You will communicate with patients and providers, lead pharmacy-related Stars measures, and help implement cost-saving pharmacy initiatives. You will oversee daily activities of pharmacy technicians and collaborate with the healthcare team to improve patient care and outcomes.
