This position ensures that care is provided in accordance with physician orders, relevant laws and regulations, and organizational policies./ppRESPONSIBILITIES:/ppClinical Care:/pulliProvides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintains and updates schedules of all disciplines in a patients home./liliConsistently assesses and evaluates the patients status, goals, and revises the plan of care as necessary. Communicates adjustments to plan of care to all team members in the appropriate time frame to support consistent delivery of care. Complete authorization modifications in a timely manner, where needed./liliImplement preventative, therapeutic, and rehabilitative procedures and maintain communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care./liliMonitors and manages patient medications; maintains an accurate medication profile; coordinates prescription refills with physicians and pharmacies and picks up medication refills as needed./liliEducate patients and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers./liliSupervises Home Health Aide, LPN, and RN to evaluate performance and quality of care./liliAccepts responsibility for assignments as qualified, ensuring appropriate training and demonstrated competency./liliUse supplies and equipment effectively and efficiently. Maintains equipment per agency policy./liliManage an assigned patient case load including tracking of patient status when transferred, following up with other healthcare facilities providing care to patients, resuming care of patients when appropriate/liliAdheres to HIPAA laws and maintains patient confidentiality./liliExecute other additional duties as assigned by the Clinical Director / Clinical management, such as performing field supervision of care team members as required by agency policies./liliCompletes and submits accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, field supervision reports, quality assurance visit reports and all other clinical documentation in accordance with Innovive company policies./lipCommunication and Meetings:/pulliMaintains consistent communication with physicians and other agencies to ensure continuity of care and implementation of a comprehensive care plan. Collaborates as part of the interdisciplinary care team./liliParticipates in scheduled clinical case management meetings, as well as vendor and referral meetings as required. Document meeting outcomes in the patients clinical record./liliCoordinates with pharmacies, insurance companies and other service providers as needed to facilitate timely patient care./li/ulpSupervisory partnering:/pulliConduct field supervision of nursing visits in a clinical management capacity while collaborating with the Clinical Director and Clinical Manager to assess the effectiveness of patient care, identify performance concerns and trends./liliConduct quality assurance visits to ensure that care provided is reasonable, necessary and in accordance with patient specific plan of care/liliPerform case load reviews with field nurses to ensure documentation supports the need for services / quick notes describe communication to care team and provider routinely and to report abnormal findings./liliMaintain and update documentation trackers for field supervision, quality assurance and case load reviews./liliEnsures the clinical team is adhering to all regulations and policies while conducting patient care in the home./liliEnsures communication and collaboration with other members of the multidisciplinary team (HHA, Rehab services)./li/ulpCOMPETENCIES:/pulliStrong verbal and written communication skills, documentation abilities, interpersonal skills, and problem-solving capabilities./liliEffective planning and organizing skills, prioritizing work activities efficiently and developing realistic action plans./liliCommitment to quality, demonstrating accuracy and thoroughness and seeking improvement opportunities./li/ulpREQUIRED QUALIFICATIONS:/pulliGraduate of an accredited school of professional nursing/liliCurrent RN license through the State Board of Nursing and meets one of the following criteria:/liliRN with a Bachelors degree in nursing and one year of related working experience/liliRN with a diploma or Associates degree with two years related work experience/liliSuccessfully passed National Council Licensure Examination (NCLEX)/liliMinimum of 1 year of clinical home health experience/liliComply with accepted professional standards and principles/liliValid driver license with an automobile that is in good working order, or other means of reliable transportation/liliGood physical stamina, must provide pre-employment physicians health clearance including a negative TB test and/or CXR and other tests as required by the organizations policy/liliPossesses and maintains current CPR certification/liliPositive references from nursing school, previous (or current) employers and/or professional peers/li/ulpPREFERRED EDUCATION AND EXPERIENCE:/pulliBachelors in Nursing (BSN) from an accredited institution/liliContinuing Education Requirements:/liliAgency personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, agency personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities.
