Elder Care Community Resource Liaison

McGregor PACE is seeking a Community Resource Liaison to bridge participants in the PACE program with community resources to help them stay healthy and independent in the community. You will coordinate with agencies, providers and social services, and support housing and other services as part of an interdisciplinary team led by the MSW/LSW.
