About this Position Behavioral Health Community Guide Location: North and Central areas of New Jersey FLSA Status: Exempt Job Status: Full Time Work Model: Hybrid Work Schedule: Monday–Friday with occasional evenings and/or weekends Vehicle Required: Yes Amount of Travel Required: 25‑50% within a 30‑mile community service area Reports To: Care Team Manager Overview Reema Health seeks a self‑motivated Community Guide to work with individuals living with serious mental illness, substance use disorder, and complex medical conditions. The role involves building relationships, coordinating care, and providing holistic support through a technology platform and in‑person visits. Responsibilities Engage members through calls, texts, and face‑to‑face interactions, building trusting relationships. Develop and manage a caseload of 60–100 members, ensuring coverage for peers during absences. Conduct initial outreach, assess member needs, and create individualized care plans. Coordinate referrals and scheduling for behavioral health, primary care, and community services. Assist members in navigating enrollment into behavioral and mental health services. Attend clinical visits as appropriate and support follow‑up on social needs. Document all interactions in the Reema App and update care plans promptly. Follow confidentiality requirements and seek multidisciplinary support for complex cases. Represent Reema positively, fostering diversity, dignity, and respect in community relationships. Support members through service transitions, maintaining self‑sufficiency focus. Utilize technology, including Microsoft Office and video‑conferencing applications. Education, Experience, and Skills Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent experience). Minimum 3 years of direct experience with populations requiring mental and chemical health support. Demonstrated crisis planning, case management, boundary and safety knowledge. Strong critical thinking, time management, multitasking, and strategic planning abilities. Proficiency in technology and basic software applications. Ability to build and maintain relationships, initiate conversations, and create connections. Qualifications & Position Requirements 18 years of age or older. Valid driver’s license, vehicle insurance, and satisfactory driving record. Pass pre‑employment drug screening and background check. Comply with HIPAA and federal, state, and local regulations regarding confidentiality. Use technology daily and lift/carry up to 25 pounds occasionally. Benefits Competitive salary.
