divh2Nurse Navigator, Emergency Medicine/h2pUnder general administrative direction, provides a significant level of patient education related to illness and planned management and treatment and facilitates coordination of care for Emergency Medicine patients. In addition, the Nurse Navigator provides in-clinic support to providers, as needed./ppEssential Duties and Responsibilities/pulliLiaison for Emergency Medicine patients requiring coordination of care and support./liliWork collaboratively with the Triage RN teams to manage clinical issues; urgent tasks, prescription pre-authorizations and refills, appointments./liliCoordinate and manage all urgent orders for dynamic testing and procedure referrals to include patient preparation and education./liliFollow up with patients on testing and procedure results in coordination with providers./liliProvide appropriate clinical support, counseling, and education. Alerts provider to urgent situations that cannot wait until scheduled follow-up./liliPrepares Clinic prep notes for new patients and returning patients which includes scanning notes/imaging, contacting referring physicians or imaging groups, and contacting patients on any issues on items needed for visit./liliPrepares records needed and reconciled for future appointments./liliAssist providers in addressing insufficiencies in care provision by coordinating follow-up, orders, and referrals./liliParticipates in quality improvement, patient safety, and patient experience initiatives./liliMeasures and tracks departmental progress in meeting quality, safety, and patient experience goals./liliParticipates in training and education of support staff./liliKeep up-to-date records/data for programs and patients within MFA and GWUH./liliIdentify and coordinate health maintenance issues./li/ulpMinimum Qualifications/pulliEducation: An Associates degree or higher in Nursing, or an equivalent combination of training and experience./liliLicensure: A current and valid D.C. license as a Registered Nurse or evidence of application for licensure in D.C./liliExperience: A minimum of 2 years of experience as a Registered Nurse./li/ulpPhysical Requirements/pulliSit, walk, reach, bend, or twist for long periods of time in a clinical setting./liliMust be able to lift, carry, push, or pull up to 20 lbs. as part of the role./liliRegularly exposed to healthcare settings that may require personal protective equipment./liliRequires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment./li/ulpEqual Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
