Full-Time Social Worker

Job Summary:

As a Full-Time Social Worker, you will play a vital role in assisting residents with social and emotional issues, ensuring their well-being and independence. We are seeking a compassionate and experienced professional to join our team and make a meaningful difference in the lives of our residents.

Responsibilities:

• Perform periodic assessments of all residents to determine status and needs

• Provide services to meet the medically related social and emotional needs of residents

• Handle resident concerns and assist residents in a respectful and non-judgmental manner, fostering independence and self-determination whenever possible

• Assist with coordinating family council

• Develop close working relationships with private and public social, health, and welfare resources in the community

• Participate in resident care conferences to assist in the implementation of care plans

• Perform assessments on new residents to obtain history, establish specific goals, and complete the appropriate section of the Minimum Data Set (MDS)

• Write progress notes according to established schedules

• Assist residents and families with adjustments following initial placement, encourage resident participation in facility activities

• Identify and document changes in affect, behavior, or personality, advise appropriate personnel

• Assist in preparing residents and families prior to discharges from the facility (when possible)

• Intervene in resident conflict situations as necessary

• Meet with residents and/or families to complete admissions paperwork, verifying completion of paperwork

Requirements:

• Minimum of an Associate’s degree in social work or in a related human service field (at minimum required); preferred bachelor’s degree in social work from an accredited college/university

• At least one year of social work experience in a healthcare facility

• Ability to read, write, and comprehend English; ability to follow oral and written instructions. Strong written, verbal, analytical, and people skills

• Fluent knowledge of Microsoft Office and other office equipment

• Ability to work hours as scheduled based on the requirements of the position assignment
