Physical Therapist (PT) - PRN - Per Visit - Home Health

Job Summary We are seeking a licensed Home Health Physical Therapist (PT) – PRN – Per Visit to join our team in Dallas, TX. The ideal candidate will have experience providing physical therapy services in patients’ homes and will be responsible for assessing patients’ needs, developing treatment plans, and delivering therapy to help patients regain mobility and independence. Responsibilities Conduct initial assessments of patients’ physical abilities and develop treatment plans. Provide therapy services to patients in their homes. Monitor patients’ progress and adjust treatment plans as needed. Communicate with patients’ physicians and other healthcare professionals to coordinate care. Document patients’ progress and maintain accurate records. Provide education to patients and their families on managing conditions and preventing further injury. Requirements Current license to practice as a Physical Therapist in Texas. Minimum of one year of experience in home health. Excellent communication and interpersonal skills.
