Qualifications
- New York NP or PA license and DEA (or eligible)
- Post-acute care experience required.
- Minimum of 1 year primary care experience
- Background in geriatrics, hospital medicine, or emergency medicine also valued
- Strong clinical assessment and independent care-management skills
