Remote Evening NP/PA: Post-Acute Care Specialist

We are seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide call coverage to our Post-Acute Facilities. This position is fully remote and can be performed from home. Shift: Evenings Qualifications: Graduate of an accredited Physician Assistant or Nurse Practitioner program Current national certification as Physician Assistant or Nurse Practitioner. Responsibilities: Provide clinical evaluation, treatment planning, and ongoing management for patients in Post-Acute Facilities during off-hours Collaborate with interdisciplinary teams to ensure high-quality, timely patient care Benefits: Eligible for insurance benefits on first day of full-time employment.
