Remote NP Per Diem — Home Health Assessments

ComplexCare Solutions (CCS) seeks a Nurse Practitioner per diem to perform comprehensive in-home and remote assessments for health plans across California. You will spend time with members to evaluate health status, identify gaps in care, and develop individualized plans aimed at keeping patients safely at home and out of the hospital.
