Patient Support Case Manager

Virtual Vocations Inc

Serving as the primary point of contact for patients and healthcare providers, the full-time Patient Support Case Manager will provide support throughout the patient journey, including pre and post prescription processes, while working remotely from Monday to Friday, 11:30am to 8:00pm EST. Key responsibilities Build and maintain relationships with patients, caregivers, and healthcare providers to facilitate access to therapy Act as the main resource for incoming inquiries, providing timely updates to stakeholders on patient cases Educate patients on their treatment journey using program-approved materials and resources Required qualifications Minimum of 5 years of pharmacy experience required Associate degree or equivalent program in pharmacy or healthcare preferred Desired certifications include PACS, CHES, or CCM Minimum of 2 years of patient support program experience preferred Pharmacy Technician certification is a plus
