Social Services Consultant - Hybrid, Full Benefits

We are seeking an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services. Key Responsibilities Include Building trusting relationships with participants Developing IRIS Support and Services Plans Maintaining ongoing contact and conducting visits Assisting with IRIS eligibility and connecting with local agencies Documenting interactions in required systems promptly Conducting annual Long-Term Care Functional Screens (as needed) Collaborating with a supportive team while working independently Qualifications & Skills Bachelor’s degree in social work, psychology, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing Strong commitment to participant autonomy and meaningful living Proficiency with smartphones, tablets, and computer systems Knowledge of human behavior, social interaction, and community resources Willingness to serve diverse populations with empathy and respect Valid driver’s license, reliable transportation, and satisfactory driving record This Role Offers Benefits such as health insurance, paid time off, holidays, retirement contributions, and wellness programs Support navigating Public Service Loan Forgiveness (PSLF) Mileage reimbursement and flexible wage options Join us to empower individuals to lead self-directed, meaningful lives through compassionate social services. #J-18808-Ljbffr Black Women's Mental Health Institute
