Work Environment

This onsite position primarily involves visiting patients in Santa Rosa and the surrounding areas, providing an intimate and focused one-on-one therapeutic environment. Key Responsibilities

• Conduct comprehensive assessments of each patient's physical abilities and limitations to develop tailored rehabilitation plans in collaboration with physicians and the multidisciplinary care team.
• Implement and oversee individualized therapy programs, adapting techniques as necessary to optimize each patient's recovery process.
• Deliver hands-on therapeutic care directly in patients' homes, employing advanced physical therapy modalities and exercises designed to enhance physical function and independence.
• Educate patients, families, and caregivers about therapy goals and methods, ensuring they are confident in executing prescribed home exercises and utilizing necessary adaptive equipment.
• Maintain accurate and detailed documentation of patient progress, modifications to treatment plans, and communicate with the care team to ensure coordinated, high-quality care delivery.
• Manage and prioritize your caseload effectively, ensuring timely and efficient patient care while maintaining compliance with relevant laws and regulations.

Patient Population / Client Focus

You will primarily serve adults and seniors who may be recovering from surgery, managing chronic conditions, or needing rehabilitation after injury. Your expertise will be crucial in helping these patients regain their independence, improve their strength, and manage their symptoms effectively, thereby enhancing their overall quality of life.

Qualifications

• Current and valid state Physical Therapy License.
• Bachelor's degree in Physical Therapy, with preference for candidates holding a Doctor of Physical Therapy (DPT) degree.
• Basic Cardiac Life Support (BCLS) certification, or the ability to obtain this certification within 30 days of employment.
• No prior experience is necessary; we welcome new graduates eager to make a positive impact in patient care.

Preferred Experience

• Experience in home health or outpatient rehabilitation settings is desirable, but not required.
