Registered Nurse (Community Care Coordinator)

Summary Registered Nurse (Community Care Coordinator) located in Community Care Services, Leeds, MA. Full‑time, 40 hours per week. Resume requirement does not apply. Qualifications English Language Proficiency (written and spoken) Graduate of a professional nursing program accredited by ACEN or CCNE, or an equivalent foreign program with a recognized equivalency evaluation. State licensure: one full and unrestricted registered nurse license from any U.S. state, territory, commonwealth, or District of Columbia. Graduate Nurse Technician exception: candidates who meet basic education but lack licensure may be appointed at entry level on a temporary basis while pursuing licensure. Grade Determinations: Level I: Associate Degree (ADN) or Diploma; no additional professional nursing required. Level II: ADN or Diploma with 1 year of specialized nursing experience OR Bachelor of Science in Nursing (BSN) with no additional experience. Level III: ADN or Diploma with 2 years of professional experience OR BSN with 1 year of specialized experience OR Master’s degree in Nursing (MSN) with no additional experience OR Master’s in a related field with BSN. Level II (Nurse II): BSN with 2 years of professional experience OR MSN with one year of specialized experience OR Master’s in a related field with BSN and one year of specialized experience OR Doctoral degree with no professional experience. Level III (Nurse III): MSN with 2 years of specialized experience OR Master’s in a related field with BSN and two years of specialized experience OR Doctoral degree with one year of specialized experience. Preferred Experience: VA Community Care, Care Coordination, Case Management, or Outpatient services. Physical Requirements: Applicants must be physically able to perform all duties; a pre‑placement physical evaluation will be completed prior to appointment. Duties Responsible for all elements of the nursing process when providing and/or supervising patient care. Assess, plan, implement, and evaluate veteran care through telephone interaction. Support delivery of high‑quality, timely care to veterans in the community. Coordinate care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care. Follow procedures per established policies and guidelines. Collaborate with interdisciplinary team members to influence care outcomes. Participate in unit/program‑level quality improvement initiatives and customer service programs. Work schedule: Monday‑Friday, 8:00 am‑4:30 pm. Telework not authorized; position is not virtual. Compensation & Benefits Salary is competitive and based on grade and step, with regular increases and potential performance awards. Paid Time Off: 50 days per year (26 days annual leave, 13 days sick leave, 11 paid federal holidays). Retirement: Traditional federal pension (5 years vesting); federal 401(k) with up to 5% employer contribution. Insurance: Federal health, vision, dental, term life, long‑term care, and other federal programs (many can be carried into retirement). Licensure requirement: one full and unrestricted license from any U.S. state or territory. This position is in the Excepted Service. Veterans’ preference does not apply for internal or current permanent federal agency employees. The position is covered by a special rate.
