What You'll Do Provide skilled nursing support across multiple home health territories throughout the Twin Cities. Complete OASIS assessments and individualized plans of care. Deliver one-on-one skilled nursing visits and patient education. Collaborate closely with physicians and interdisciplinary team members. Support local teams during staffing needs, growth, and coverage. Maintain accurate and timely clinical documentation.

What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical assessment and critical thinking skills. Comfortable adapting to different territories and patient populations. Home health experience preferred, but not required. Reliable transportation and willingness to travel throughout the metro.
