Case Manager

Registered Nurse – Case Manager

Location: Lake & Marion County, FL (Ocala area)

Schedule:

• Monday–Friday
• 8:00 AM – 5:00 PM

Travel Required: Between multiple skilled nursing facilities

Dress Code: Medical attire (scrub color not specified)

Primary Responsibilities:

• Ensure accurate and complete documentation in compliance with clinical and regulatory standards.
• Collaborate with the interdisciplinary care team to support patient care and coordination.
• Assist providers with care coordination activities, including benefit determination for Medicare Part A, B, and D.
• Partner with the primary Advanced Practice Clinician (APC) and facility staff to identify and respond to changes in patient condition.
• Support therapy coordination and participate in facility-based meetings and family conferences.
• Coordinate training, track compliance, and support quality measure initiatives.
• Conduct and assist with in-service training for selected audiences.
• Aid providers in documentation, coding, medication reconciliation, nursing rounds, and assessments.
• May write verbal orders from providers in compliance with state RN practice laws and facility protocols.
• Assist in managing patients during acute medical episodes or short-term sub-acute rehabilitation.

Required Qualifications:

• Active and unrestricted RN license in the state of residence (or ability to obtain by start date).
• Associate degree or higher in Nursing.
• Certified in Basic Life Support (BLS).
• Ability to lift up to 30 pounds and perform physical assessments.
• Access to reliable transportation.
• Fully vaccinated against COVID-19 (compliance with federal, state, and client requirements).

Preferred Qualifications:

• Minimum 1 year of clinical experience (long-term care setting preferred).
• Experience with patients who have complex behavioral, social, or functional needs.
• Understanding of geriatrics and chronic illness management.
• Proficient in electronic medical records and documentation.
• Strong communication and time management skills.
• Ability to work with diverse care teams across multiple facilities.
• Willingness and ability to travel between facilities during shift hours.
