Medical Care Coordinator

to join our dedicated team and help our residents bridge the gap between treatment and long-term stability. As a Medical Care Coordinator, you are the person who ensures clients make their appointments, get their vital medications, and access critical life resources. Watch the real, first-hand progress of individuals as they rebuild their health, confidence, and independence. Key Accountabilities: Assist clients in accessing ongoing services, including medical, dental, vision, housing, financial assistance, and legal advocacy. Support clients in obtaining or reactivating disability benefits, Medicaid, and other needed resources. Ensure timely medication distribution and support medication compliance. Conduct daily medication checks and document compliance in the EMR system. Monitor new or updated medications to ensure appropriate documentation and continuity. Complete and upload all incidental funds paperwork with accuracy and adherence to agency policies. Assist counselors with discharge planning. Perform additional duties as assigned. Essential Requirements: High School Diploma required, bachelor’s degree preferred. 1 year of Care Coordinator experience. 2 years of Behavioral Health experience. Experience working with individuals with substance use disorders preferred. Strong communication and problem‑solving skills. Ability to work effectively with a diverse population. Computer literacy and comfort with EMR systems.
