The Nurse Director of Case Management will lead a friendly team of nurses, social workers, and utilization specialists across interdisciplinary rounds; optimize discharge planning and transitions of care; reduce length of stay and readmissions; manage denials and payer relations; ensure CMS and Joint Commission compliance; mentor emerging leaders; and leverage decision-support tools to deliver timely, patient-centered, and financially responsible care in a diverse, collaborative setting.
