Full Time Charge Nurse, RN - Post Acute

Overview The post-acute Charge Nurse leads the daily clinical operations of the dialysis treatment floor. Serves as the on-site clinical leader for the dialysis program within the Skilled Nursing Facility when the Area Manager is not present, ensuring safe operations, regulatory compliance, and high-quality patient care. May supervise other clinical staff to ensure safe, quality, and efficient care is being delivered. Responsible for completing daily assignments to ensure the safe, efficient, and quality care of the patients. In the post-acute dialysis setting, the Charge Nurse plays a key leadership role in ensuring the safe and efficient operation of the dialysis program within the Skilled Nursing Facility. Responsibilities Serve as the on-site clinical leader of the dialysis program during assigned shifts. Provide clear direction and clinical guidance to dialysis technicians and other assigned staff. Promote accountability and adherence to Rendevor Dialysis policies, procedures, and safety standards. Maintain strong communication and collaboration with Skilled Nursing Facility nursing staff and leadership. Identify operational or clinical concerns early and elevate issues appropriately to the Area Manager or leadership team. Foster a culture of teamwork, patient safety, and continuous quality improvement. Responsible for daily clinical activity on the treatment floor. Reports to and works closely with the area manager or director. Reviews daily assignments for all staff on the treatment floor to ensure proper, safe, efficient, and quality treatment and care of patients. Provides clinical leadership and direction to dialysis technicians and other assigned staff during dialysis operations. Ensures staff follow established policies, procedures, and safety protocols. Performs thorough patient assessments as evidenced by documentation and feedback to their manager and/or nephrologist as needed. Recognizes changes in patient status while on dialysis and makes necessary adjustments to treatment. Appropriately responds to all emergencies, notifies the nephrologist when necessary and seeks direction, and obtains orders. Draws upon professional nursing knowledge to set appropriate priorities for patient care.? Consistently reviews and evaluates priorities as changes occur and conveys these priorities to appropriate staff members. Develops, implements and evaluates appropriate plan of care for patients based upon patient health status, co-morbidities, current lab results, and response to treatment. Utilizes the team approach by involving the patient, nurses, technicians, nephrologist(s) social worker, and dietitian as appropriate and consistent with institutional policy. Assists in building and maintaining a facility/clinic culture of safety and quality. Delegates CMPA and POC assignments to other nurses as needed. Ensures all forms and patient documents are completed in a timely manner. Demonstrates excellent communication skills in a clear, concise, and accurate manner. Knows and follows all Rendevor Dialysis policies and procedures. Ensures dialysis services are provided in compliance with CMS Conditions for Coverage, infection control standards, and applicable state and federal regulations. Utilizes resources to improve skills, knowledge, and practice to assure continuous quality patient care. Actively participates in Quality Assurance and Performance Improvement/Continuous Quality Improvement. Participates in the IDT and QAPI/CQO committees and attends meetings as needed and/or requested. Identifies and reports concerns, issues, and opportunities for improvement. Participates in performance improvement projects within licensure, certification, and scope of practice. Participates in annual QAPI/CQI training and other continuing education, as assigned, and required. Provides suggestions for alternate strategies for indicators not improving. Reports errors and near misses so that steps can be taken to prevent recurrence. Reports any negative feedback, complaints, grievances from other staff members, patients, and families. Uses evidence-based guidelines and proper infection control practices. Makes rounds daily on all patients completing assessments, checking machine settings, maintaining open communication, and verifies the accuracy of all orders during treatment. Ensures dialysis treatments are delivered according to the prescribed schedule and coordinates with Skilled Nursing Facility staff to address patient readiness, treatment delays, or missed treatments. Performs patient care as assigned and when indicated or needed. Acts as a communication bridge between the unit leader, dialysis staff, and patients. Serves as the primary clinical liaison between Rendevor Dialysis and Skilled Nursing Facility nursing staff and leadership during dialysis operations. Objectively assesses problems regarding staff and/or patients and works toward a swift resolution. Communicates with staff and patients regarding policies and procedures. Documents in a clear, concise, and thorough manner. Sees that all documentation is completed properly by all staff during shift. Administers medications when needed and ordered; understands and teaches principles of safe dosage in ESRD, reason for giving, and possible side effects and ensures that medications are given properly and as ordered. Ensures lab testing is completed per orders and demonstrates a strong understanding of the implications of common lab results as they apply to the ESRD patient; communicates with the nephrologist and other members of the patient care team. Promotes the education of the patients and the understanding of their disease and its treatment. Promotes a positive and productive atmosphere. Develops and/or participates in in-service education. Involved in the planning and implementation of orientation of new staff. Promotes a safe, neat, and clean environment. Assists Unit Leader in implementing and enforcing cost‑saving measures and controlling waste. Reports any incidents to Area Manager and/or Director. Checks email regularly and responds appropriately and timely; uses company‑provided email for all work‑related email communications. Maintains regular and predictable attendance and punctuality. Adheres to all company policies and procedures. Performs other duties as required and/or assigned. Qualifications Education and Experience: Graduation from an accredited nursing school or equivalent Current state license as a registered nurse Current CPR certification 18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months. One-year of clinical dialysis experience required Required Skills and Competencies: Ability to satisfactorily communicate in English including reading, writing, speaking, and understanding Strong technical/computer proficiency with laptops, tablets, Outlook, Excel, PowerPoint, Word, SharePoint, etc. Supervision This position will report directly to the Area Manager or National Director. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms. This position requires the ability to push, pull or lift supplies and products up to 50 pounds. Specific visual abilities required by this job include close vision and ability to adjust focus. The employee is required to work the full schedule which may range from five to fifteen hours in any given day, based on the unit requirements (i.e. patient census, staffing needs, etc.).
