Responsibilities
- Obtain medication histories from patients and gather relevant home medication information
- Reconcile home medications with those prescribed during hospitalization
- At discharge, identify medication discrepancies and communicate findings to prescribers for reconciliation
- Perform standard, structured medication reconciliation tasks as part of the care process
- Document and communicate medication-related issues to the care team to support safe transitions of care

Requirements
- Experience with medication histories and reconciliation or pharmacy workflow preferred
- Ability to communicate clearly with patients and prescribers
- Strong attention to detail and accuracy in documentation
- Comfortable working in a hospital setting and collaborating with a multidisciplinary team
- Availability for a varied shift schedule as part of a travel assignment
