Population Health Coordinator

Population Health Coordinator Key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health. Documents all patient interactions in a concise manner that is compliant with documentation requirements Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management Assists to schedule and coordinate patient transportation, follow-up physician appointments Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports Provides excellent customer service by serving as a resource to all internal and external customers Maintains knowledge of all health plan benefits and payor requirements Call patients as designated by PCP or clinical staff Verify patients have been seen and follow up on no show patients Assist all patients with barriers Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of care Coordinates other medical office assistants in outreach and report validation needs Closes Care Gaps Minimum Qualifications: High school graduate or GED equivalent preferred More than one year of related experience in a medical setting Basic computer literacy Knowledge of medical terminology Good communication and customer service skills Able to perform all Medical Office Assistant job functions and responsibilities Required Licensure/Certification Skills: Valid NYS drivers' license Preferred Licensure/Certification Skills: Current Medical Assistant certification or registration preferred Rochester Regional Health is an Equal Opportunity / Affirmative Action Employer. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
