Job Description:

Provides physical therapy services to patients/clients according to a written physician's plan of care. Responsibilities include assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care.

Responsibilities:

• Assessing, evaluating, and updating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
• Evaluates home environment. Assesses for muscle strength, mobility, gait, ROM - potential for rehab. Directs physical therapy treatment.
• Instructing patients/clients and families/caregivers in the use and care of therapeutic appliances. Determining priority needs for physical therapy.
• Reporting the patient's/client's reaction to treatment or changes in condition to the physician.
• Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy.
• May train patient/client in the use of prosthetic devices.
• Supervising homemaker/Home Health Aides when such personnel are participating in patient's/client's rehabilitation regime.
• Identifies patient/client and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals, observing, recording and reporting information.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Participates in peer consultation process.
• Participates in and completes ongoing staff competency assessment.
• Complies with and understands Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other regulations and licensure requirements.
• Performs other duties as assigned and/or requested.

Requirements:

• Minimum of 2 years of experience as a PT in an acute/home care/rehab setting.
• BLS Certification required.
• Good oral and written communication skills; has good organizational skills.
• Scrubs are not required. If not worn, the dress code is slacks and a polo or button-up shirt.
• Candidates may not have been directly employed by any facility associated with CommonSpirit within the past year.
