• Conduct comprehensive assessments of patients' mental health, identifying strengths, challenges, and overall needs.

• Develop individualized treatment plans based on clinical assessments and evidence-based practices.

• Provide evidence-based therapeutic interventions, including individual counseling, group therapy, and family support sessions.

• Document patient progress accurately and timely in our electronic health record (EHR) system, ensuring compliance with regulatory standards.

• Collaborate with other healthcare professionals, such as psychiatrists, nurses, and occupational therapists, to create an integrated care experience for patients.

• Educate clients and their families about mental health conditions, coping strategies, and available community resources.

• Stay informed on advancements in the mental health field, integrating new research into practice.

You will be serving a diverse patient population at our hospice center, which includes individuals facing serious illness and their families. Many patients experience complex emotional and mental health issues related to their health conditions, making your role as an LCSW crucial. Your expertise will help them navigate these challenges, providing therapeutic support that enhances their quality of life during a pivotal time.

• A Master’s degree in Social Work from an accredited institution is a prerequisite.

• A valid LCSW license in the state of Tennessee is required to practice.

• Demonstrated excellent communication skills, both verbal and written, are necessary for effective patient interactions.
